Dr. Guy Cappuccino is an expert in breast surgery in Mount Airy, Maryland that provides various breast procedures including breast augmentation, breast lift, breast reduction, and more. The Breast Augmentation Surgery Itself

Breast augmentation can be performed with general anesthesia, so you'll sleep through the entire operation. Prior to incision, Dr. Cappuccino places two sets of ultrasound guided blocks to improve surgical outcomes and minimize post-op pain. He places a PEC1 block (relaxes the chest muscles) and SAP block (serrates anterior plane) which numbs the breast itself. The method of inserting and positioning your implant will depend on your anatomy and Dr. Cappuccino’s recommendation. The incision can be made either in the crease where the breast meets the chest or around the areola (the dark skin surrounding the nipple).

Working through the incision, Dr. Cappuccino will lift your breast tissue and skin to create a pocket, either directly behind the breast tissue (submammary or subglandular placement), beneath the pectoral muscle and on top of the chest wall (submuscular placement), partially behind the muscle (partial-submuscular), or below the covering of the muscle only (sub-fascial). Once the implant is positioned within this pocket, the incisions are closed with sutures, skin adhesive, and/or surgical tape. A gauze bandage may be applied over your breasts to help with healing. The surgery usually takes one to two hours to complete.

After Your Breast Augmentation Surgery

You're likely to feel a little sore for a few days following your surgery, but you'll be up and around in 24 to 48 hours. Dr. Cappuccino will ensure that you have the medication required to manage discomfort and most patients use only Tylenol and Motrin.

Within 24 hours, any gauze dressings will be removed and you may shower. A surgical bra will be placed after surgery to help with the healing process. You may experience a burning sensation in your nipples for about two weeks, but this will subside as bruising fades.

Your stitches will dissolve. There will be swelling in your breasts after surgery that may take three to five weeks to disappear. During your consultation, Dr. Cappuccino will give you a very clear idea of your recovery process.

The Best Candidates for Breast Augmentation

Breast augmentation is a transformative procedure that can enhance both the size and shape of the breasts, providing a boost in confidence and self-esteem. Candidates for breast augmentation typically include:

  • Women who desire fuller breasts or wish to correct asymmetry.
  • Individuals who are physically healthy and at a stable weight.
  • Those with realistic expectations about the outcomes of the procedure.
  • Women with fully developed breasts, usually over the age of 18.
  • Individuals who are dissatisfied with the size or shape of their breasts due to factors such as pregnancy, weight loss, or aging.

If you're considering breast augmentation, schedule a consultation with a board-certified plastic surgeon to discuss your goals and determine if you're a suitable candidate for the procedure.

Considering Various Implant Features

The size of a breast implant is measured in cubic centimeters (ccs) based on the volume of the saline or silicone filler. Breast implants vary both by filler and in size, but there are additional features to consider:

  • Texture: the implant shell may be smooth or textured
  • Shape: the implant may have a round profile or one that is more anatomic (teardrop or tapered shape)
  • Profile: the implant may have a low, medium, or high projection (the depth of the implant from the base to the highest point of the implant curve)
  • Diameter: the width of the implant measured across its base (the side of the implant that will be positioned over the chest wall)

What are the different types of Implants?

The choice of implant filler, implant size, shape, and other features will be determined based on your breast anatomy, body type, and your desired increase in size. Your lifestyle, goals, and personal preferences, will also be determining factors. Dr. Cappuccino offers a choice between silicone and saline implants. During your preoperative consultation, Dr. Cappuccino will talk you through the features of each implant type, and help you make the choice that suits you best.

Breast implants are medical devices with a solid silicone, rubber shell which can be filled with either saline solution (sterile salt water) or elastic silicone gel. Both saline and silicone gel breast implants are approved by the U.S. FDA, meaning they have been rigorously researched and tested, and reviewed by an independent panel of physicians for safety.

Patients should wait until their breasts are fully developed to consider augmentation. Saline implants are FDA approved for augmentation in women 18 years of age and older. Silicone implants are FDA approved for augmentation in women age 22 and older. Saline or silicone implants may be recommended at a younger age if used for reconstruction purposes.

Living with Breast Implants

You should be aware that breast implants are not guaranteed to last a lifetime and future surgery may be required to replace one or both implants. Regular examinations for breast health and evaluating the condition of your implants are important whether you have chosen saline or silicone breast implants.

Dr. Cappuccino takes a particularly patient-centric approach and will work closely with you to guide you through each and every aspect of your cosmetic journey. After your surgery, Dr. Cappuccino and his team will set up a series of follow-up appointments. How the Keller Funnel™ Improves the Breast Augmentation Procedure

Dr. Cappuccino’s goal is always to make the smallest possible incision—and resulting scar—during breast augmentation. That is one reason why he uses the Keller Funnel, a patented device that helps ensure optimal safety and beautiful results.

Reduced Incision Lengths

Using the Keller Funnel enables Dr. Cappuccino to make smaller incisions, similar to those used for saline implants.

Increased Sterility

The Keller Funnel maximizes sterility by reducing the amount of handling necessary to insert the implant. There is less chance of bacterial contamination if the implant is touched less.

Increased Options for Incision Placement

Easier insertion through smaller openings offers more opportunities to conceal the incisions. This is especially useful for women who may scar easily.

Minimize the Potential for Implant Damage

According to an engineering study conducted by Keller Medical, Inc., the Keller Funnel uses less force than traditional implant delivery methods. This creates less stress on the implant shell, which reduces both short- and long-term complications associated with mishandled implants.

Potential Breakage or Leaking

Occasionally, breast implants may break or leak. Rupture can occur as a result of injury or even from the normal compression and movement of your breast and implant, causing the man-made shell to leak. If a saline-filled implant breaks, the implant will deflate in a few hours, and the saltwater will be absorbed and naturally expelled by the body.

If a silicone-gel-filled implant leaks or breaks, the newest generation, highly cohesive silicone gel usually remains within the implant shell. A ruptured implant filled with silicone gel may not deflate and may not be noticeable except through imaging techniques such as an MRI. In either case, it’s important to consult your surgeon to discuss the appropriate steps for repair or replacement.

Breastfeeding with Implants

Some women report that their nipples become oversensitive, undersensitive, or even numb after undergoing a breast augmentation procedure at Dr. Cappuccino’s Maryland operating facility. You may also notice small patches of numbness near your incisions. These symptoms usually disappear within time, but may be permanent in some patients.

Breast implants do not generally interfere with a woman's ability to breastfeed or present a health hazard during pregnancy to a woman or her baby. However, pregnancy and the associated changes to a woman's body can potentially alter the results of any breast surgery. If you are interested in becoming pregnant and breastfeeding in the future, it’s important to discuss this aspect of your breast augmentation in Frederick, MD with Dr. Cappuccino during your preoperative consultations so he can thoroughly guide you through what to expect.

Recovery after Breast Augmentation

You should be able to return to work within a few days, depending on the level of activity required for your job. Every procedure is different, so Dr. Cappuccino will give you specific instructions for your recovery period. Your breasts will probably be sensitive to direct stimulation for two to three weeks, so you should avoid much physical contact. After that, breast contact is fine once your breasts are no longer sore, usually three to four weeks after surgery.

Your scars will be firm and pink for at least six weeks. Then they may remain the same size for several months, or even appear to widen. After several months, your scars will begin to fade, although they will never disappear completely. Routine mammograms should be continued after breast augmentation for women who are in the appropriate age group, although the mammography technician should use a special technique to assure that you get a reliable reading, as discussed earlier.

Breast Augmentation FAQs:

How much does a Breast Augementation cost?

The cost of a breast augmentation procedure can vary based on factors like the type of implants used, the surgeon's expertise, and the geographic location of the clinic. On average, the cost may range from $6,600 – $7,200. It's essential to consult Dr. Cappuccino to determine the specific costs and options that align with individual preferences and goals.

Will breast augmentation increase my risk of breast cancer?

This is one of the most frequently asked questions by women considering this surgery. Many studies have shown that women who get implants are not at an increased risk for getting breast cancer. In addition, implants do not delay the detection of breast cancer. Numerous studies, as well as evidence from the National Cancer Institute, show that having implants does not hinder the patient or their physician in finding breast tumors. Finally, breast implant patients who do get breast cancer have the same chance of obtaining remission, or being free of cancer for 5 years, as women without implants.

Am I too old for breast augmentation surgery?

Many women in their 40s and 50s have undergone breast augmentation surgery with good results. The most common patient for this surgery is a woman in her 30s with two children who has been considering this operation for many years. Whatever your stage in life, we encourage you to schedule a consultation to find the best path toward your cosmetic goals.

Should I wait until I am done having children to have augmentation?

Pregnancy will change the size and shape of a woman's breasts whether she has implants or not. These changes may adversely affect the cosmetic appearance of any augmented breast. It is usually recommended that pregnancy be postponed until 6 months after the surgery. Breast-feeding with implants is still possible and has been shown to be safe for the baby and the mother.

How do I choose my new size?

During your consultation, your plastic surgeon will work with you to identify what size of breast you desire from augmentation. "Breast size" as measured by bra-size is variable and is often not a good way to measure final size. As a general rule, every 125-150cc of implant volume equals an increase in a single cup size. However, every patient's body is different. By working with your surgeon and their clinical team, you can set obtainable and healthy expectations together. It is helpful for you to bring in a bra the size you would like to be to help in finding the right sized implant for you.

How long until I can exercise after my breast augmentation?

It is recommended that patients begin walking immediately after surgery. However, women should not perform any intense physical exercise for six weeks following the operation. Physical exercise including weight lifting, biking, jogging, and other forms of intense activities may cause implants to shift position or cause wound healing problems that may alter the appearance of the breasts after surgery.

When can I return to work after my breast augmentation?

This depends on your job and its physical demands. Some general guidelines are: Upper arm movements, like reaching, should be avoided for the first 1-2 weeks. Lifting anything heavier than 5 pounds should be avoided for 6 weeks after the operation. This lifting restriction may prevent some women from returning to work.
